It is estimated that 19 million people in the USA consume fish oil preparations, the global market comprises 31 billion US dollars. ASCEND is the largest and longest lasting placebo-controlled randomized trial for the supplementation of omega-3 fatty acids. This included 15,480 patients over a period of seven years and resulted in:

• No impact on the primary outcome of severe vascular events
• No impact on cancer, total or cause-specific mortality

The guideline recommendations should be reconsidered in light of the study results.
